If you have already been trained as a surrogate partner and want to apply for certified surrogate partner member status, please complete these steps:
- Pay this application fee.
- Complete the online surrogate partner certification application questionnaire (link will be emailed to you after you fill out the form listed above).
- Submit two references from therapists you’ve worked with on surrogate partner therapy cases, and additional references from clients, using our online form (link will be emailed to you after you fill out the form listed above).
After all of the above steps are complete, we will contact you to schedule an interview.
